二、理赔过程中常出现问题的材料：

Documents that often cause issues during the claim application process：

1.医疗费用发票原件（必须提供）

The original medical expense invoice(mandatory)

发票获取方法：请您扫描缴费凭条上的二维码下载电子发票，带有医院收费红章的发票才是合格的医疗费用发票，也可以在医院交完费用就到窗口或自助打印机打印发票。❌️不要上传您手中的缴费凭条，挂号单，检查申请单，微信或者支付宝的支付记录等等。

Way to aquire the invoice：Actually you can scan the QR code on the payment receipt to download the electronic invoice. Only the invoice with the hospital's red seal for charges is a qualified medical expense invoice, You can also print the invoice at the service window immediately after payment at the hospital. ❌️Do not upload the payment receipt, registration slip, examination application form, We-chat or Alipay payment records, etc.

2.病历（必须提供）

Medical record(mandatory)

门诊/急诊病历获取办法：病历是由医生书写的内容，每次看病都务必让医生打印病历。

Way to aquire the Outpatient/emergency department medical record : The medical record is the content written by the doctor, ask the doctor print medical record for every single time's visit.

如果有多次就诊，需要就诊发票日期与病历日期一一对应。❌️诊断证明、检查报告、处方等不能代替病历。

If you visited the doctor many times, please make sure the visit date on the invoice correspond with the date on the medical record. ❌️Diagnosis certificates,examination reports, prescriptions, etc. cannot replace medical records.

3.当意外发生事故时请同时准备意外事故说明(必须提供)

In case of an accident, please prepare an accident report (mandatory)

意外证明获取方法：请将我们发给您的意外证明模板打印出来，按照要求填写相关内容。

Way to aquire the accident report：Please print out the accident report template we sent to you and fill in the relevant information as required.

(如发生的是交通事故，则需要提供交通管理部门出具的交通事故责任认定书；如果涉及纠纷，出具公安部门的相关证明)。

Accident certificate (mandatory), you can download the template to fill out: Accident Certificate.

(If the injury refers to traffic collision, A traffic accident liability determination document issued by the local traffic management department is required;

Moreover, if you are in dispute with the third party, then the relevant certificates issued by the public security department should be provided.)

4.银行收款信息

Bank account information

银行账户信息获取方法：需到银行柜台打印能看到卡号，户名和开户行这三个信息的银行回单或者APP截图，❌️只提供银行卡照片是不行的。

Way to aquire the Bank account information：Please go to bank print the bank holder’s account information statement or a screenshot of the bank APP showing the card number, account name and the opening bank.

❌️A photo of the bank card is not enough.
